PET QUALITY OF LIFE ASSESSMENT +/- EUTHANASIA

This service is offered for those families who would like a quality of life consultation to assist with guidance on quality of life assessment and decision making. If a quality of life assessment is performed and euthanasia is not elected families are charged for a quality of life assessment only.
